This month I want to share with you some history about this newsletter. As a patient safety foundation that was created to connect the dots between stakeholders in healthcare, this monthly email gives you a window into the Patient Safety Movement Foundation, including our upcoming events, webinars, new partners, and our Spotlight.
Our Spotlight was created specifically to highlight success stories we’ve collected from the hospitals and partners in our network who are significantly reducing preventable death and helping us get closer to ZERO by 2020 (0X2020). We hope that you, as a reader, will look forward to our newsletter each month, to see how hospitals in our network are implementing processes to eliminate preventable deaths, and share their journey with the larger community.
Our Spotlight this month highlights two hospital success stories, both implementing Hand-off Communications (HOC) checklists. It’s estimated that 80% of serious medical errors involve miscommunication between caregivers during patient transfers. We highlight Parrish Medical Center’s early data from implementing an emergency department to medsurg unit checklist and UCI Medical Center’s experience over the years, implementing new hand-off communication tools.
There is an actionable solution for hospitals to reduce harm and mortality associated with Hand-off Communications. Please download and implement it; in the Actionable Patient Safety Solutions (APSS) there are 11 checklists that can be adapted within your hospital.
