2012-2020 Initiative

The PSMF’s 2012-2020 initiative was a voluntary, free initiative in which hospitals made a commitment around one or more of our Actionable Patient Safety Solutions (APSS). These commitments were designed to showcase how a hospital or healthcare institution is improving safety and solving a patient safety challenge. Hospitals and healthcare organizations self-reported how many lives they believed to have saved through their commitment, updating the PSMF annually. The initiative began in June 2012 and ended in September 2020. We now ask hospitals and other healthcare organizations across the continuum of care to make a commitment to ZERO by becoming highly reliable. Learn more about the new initiative here.

5-Star Hospital Program

Beginning in 2018 the Foundation recognized hospitals that made commitments in alignment with the PSMF’s 18 challenges and 34 solutions. Those hospitals that gained enough points would be recognized as a 5-Star Hospital and could use this recognition to show their commitment to improving patient safety within their institution.
