Care Coordination During Transitions of Care

The Patient Safety Movement Foundation is joined by panelists representing the family member, physician, translational researcher, and nursing leadership perspectives from the US, UK, and UAE. The panel will begin discussions by examining typical gaps in care coordination across the globe, but will then take a deeper dive into:
1) The strategies everyone in the organization can take to meaningfully embrace patients and family members as equally valued members of the care team,
2) The significant foresight needed by everyone to evaluate the role of social determinants in care coordination, and
3) The needed preoccupation with areas for improvement in workflows from the perspective of the patient.
This activity has been approved to provide 1.0 continuing education credit for physicians (CME), nurses (CNE), pharmacists (CPE), certified professionals in patient safety (CPPS), healthcare executives (ACHE), patient advocates (BCPA), and certified professionals in healthcare quality (CPHQ).
